However, in the diagnosis of Non-Celiac Gluten Sensitivity, science has begun to look beyond this protein, raising fundamental questions about what other wheat components might be triggering an immune or inflammatory response in individuals who, having ruled out celiac disease and wheat allergy, still feel unwell after eating wheat derivatives.

Is gluten really the only culprit?

Modern research suggests that wheat is a complex food. Beyond the prolamins that make up gluten, there are other proteins and biological compounds that could be behind the discomfort. Among them, amylase-trypsin inhibitors (ATIs) have gained special prominence in recent clinical studies. These proteins, whose natural function is to protect the grain from insects and pests, have been shown to have the ability to activate receptors in our immune cells, which could trigger gut inflammation.

<picture class="gt-avif-picture"><img data-no-lazy="1" loading="lazy" data-gt-avif="1" decoding="async" width="1024" height="576" src="https://glutendtrotters.com/wp-content/uploads/2026/07/Foto-noticia-Sensibilidad-al-Gluten-no-celiaca-1024x576.jpg" alt="drawing of an intestine. Many diagnosed individuals with non-celiac gluten sensitivity experience significant improvement upon removing wheat, but the reduction of these FODMAPs may well be partly responsible for the disappearance of symptoms such as bloating, gas, and chronic abdominal pain.

The complexity of a clinical diagnosis

Diagnosing this condition, often linked to general gluten intolerance, is a process of exclusion. By not presenting the typical villous atrophy of celiac disease nor the IgE-mediated allergic response, patients often feel disoriented. Understanding that the body’s response can be multi-causal is vital to improving the clinical management of the condition. It is not just about eliminating a protein, but about recognizing the interaction between different wheat elements and our gut microbiota.

It is important to highlight that, for those living with this sensitivity, the recommendation remains a wheat-free diet until symptoms subside, always under professional supervision. Science continues to advance to determine whether, in the future, we will be able to differentiate which subgroup of patients reacts to gluten and who reacts to fructans or ATIs, allowing for a more personalized and less restrictive diet.

Nutrition is a fundamental pillar in our community’s quality of life. Staying informed about these scientific advances allows us to manage our health with greater rigor and safety.

Source: What wheat components cause non-celiac gluten sensitivity?

When discussing digestive discomfort after consuming grains, gluten is usually the primary suspect.
