It builds on a 2024 study by the same team, published in Nature Immunology, which had already shown that the drug protects the small intestinal mucosa from gluten-induced damage. This new study is the first to look at its effects on the body as a whole.

What does the drug actually do?

The drug being tested, called ZED1227, works by inhibiting an enzyme the body produces called transglutaminase 2 (TG2). In people with celiac disease, this enzyme modifies gluten in a way that triggers the immune system’s inflammatory response. By blocking the enzyme’s activity, the drug reduces that harmful immune reaction.

The study involved adults with celiac disease who had long been following a strict gluten-free diet. Over six weeks, participants were exposed to small amounts of gluten, combined either with the drug or with a placebo. Researchers then analysed blood samples for lipid metabolism, proteins and DNA methylation, and compared them with tissue samples taken from the small intestine.

The results showed that gluten caused clear changes in lipid metabolism in the placebo group, while in those taking the drug, those changes were almost completely suppressed. The drug also helped restore blood protein and epigenetic profiles closer to what’s typically seen on a gluten-free diet.

<picture class="gt-avif-picture"></picture>
Foto de National Institute of Allergy and Infectious Diseases en Unsplash

A finding that goes beyond the gut

Until now, this type of drug was known to protect the intestinal lining. What’s new here is that its effects appear to extend well beyond the gut — something the researchers themselves describe as a scientifically significant discovery.

“Our study published in 2024 showed that the drug effectively protects the intestinal mucosa. We now demonstrate that its effects extend to the whole body,” says Associate Professor Keijo Viiri of the University of Oulu, the study’s corresponding author.

According to the researchers, drugs like this could eventually complement dietary treatment — particularly for people who are inadvertently exposed to gluten (through cross-contamination, for example) or who keep experiencing symptoms despite following a strict gluten-free diet. Viiri’s research group is now investigating how inflammation-related damage to the intestinal lining could be prevented or repaired through drug therapy, with the aim of strengthening protection especially for higher-risk patients.

Does this replace the gluten-free diet?

No — not yet, and maybe not ever. The study is part of an international clinical trial still in the research phase, and the team itself frames the drug as a possible complement to the gluten-free diet, not a replacement for it. Today, a strict gluten-free diet remains the only effective treatment for celiac disease, an autoimmune condition in which gluten triggers an inflammatory response in genetically susceptible people. In Finland, one of the countries with the highest prevalence in the world, around 2.4% of the population is estimated to have the condition.

It’s also worth keeping in mind that this is an experimental drug still in clinical research, funded in part by the pharmaceutical company Dr. Falk Pharma GmbH and by the University of Oulu’s Profi8 Health Dimensions research programme. Any eventual approval and public availability is still a long way off.

Why this matters if you live gluten-free

Many people with celiac disease live with a constant low-level worry about cross-contamination — that small trace of gluten that can slip through in a poorly handled “gluten-free” dish, or in a mislabeled product. A drug capable of softening the impact of those accidental exposures, without replacing the diet itself, could offer an extra safety cushion for people who are already doing everything right. Still, this is early-stage science — it will take much more research before anything like this reaches a doctor’s office.

Source: University of Oulu, Finland, official press release, and original study: Dotsenko, V., Le, H.H., Rajić, S. et al. “Therapeutic TG2 inhibition reverses systemic multiomic dysregulation in celiac disease”, BMC Medicine (2026), DOI: 10.1186/s12916-026-04892-y.

What is transglutaminase 2 (TG2) and why does it matter in celiac disease?

Transglutaminase 2 is an enzyme the body produces that modifies gluten in a way that triggers the immune system’s inflammatory response in people with celiac disease.
